Booking

Do I need to book an appointment in advance?
We strongly recommend booking in advance to secure your preferred date and time, but we also accept walk-ins at all 7 clinics when we have availability on the day.
Can I get a same-day appointment?
Yes — SWDM regularly offers same-day appointments, especially at our Cardiff clinic. Call 02920 30 90 76 or use the online booking form to see today's availability.
How do I cancel or reschedule?
Contact us by phone on 02920 30 90 76 at least 24 hours before your appointment. Rescheduling within 24 hours may incur a small admin fee.
Which clinic should I book?
We have clinics in Cardiff, Newport, Swansea, Mountain Ash, Bridgend, Port Talbot, and Bristol. Choose whichever is most convenient — every clinic offers the same services at the same price.

What to Expect

What happens at a driver medical appointment?
Your appointment includes a vision test, blood pressure check, cardiovascular screening, a review of your medical history, and D4 form completion. Most appointments take 10 to 15 minutes.
What should I bring?
Bring your completed D4 form (printed from gov.uk), your current driving licence, glasses if you wear them, and a list of any regular medications.
How long does the appointment take?
Around 10 to 15 minutes for the examination itself, plus a few minutes for form completion. Most drivers are in and out within 20 minutes.
Will I get my D4 form on the day?
Yes. Dr. Al-Hassan completes your D4 form during the appointment and hands it to you to submit to the DVLA. There is no wait.
Is the examination physical or invasive?
No — it is a general health check covering vision, blood pressure, heart, and a review of your medical history. There is no undressing or physical examination of private areas.

DVLA

Does my medical get sent to the DVLA automatically?
No. You submit your completed D4 form to the DVLA yourself, either by post or online alongside your licence application.
How long is a DVLA medical valid for?
For drivers under 45, typically 5 years. For drivers 45 to 65, every 5 years. For drivers 65 and over, annually. Your DVLA licence will show your next medical date.
Can my GP do my D4 medical?
Yes, but most GPs charge £100 to £200 and waits can be weeks. SWDM offers the same examination from £55 with same-day availability and doctors who do this every day.
Do I need a D4 for a taxi licence?
You need a Group 2 medical (the same examination the D4 form captures), but your council may use their own form instead of the D4. We complete whichever form your council requires.

Medical Conditions

I have high blood pressure — will I fail?
Not automatically. The DVLA permits a driving licence up to a specific threshold and with appropriate treatment. Dr. Al-Hassan will measure your blood pressure on the day and advise you based on DVLA guidance.
I have diabetes — can I still drive professionally?
In most cases yes, subject to DVLA rules around insulin, awareness of hypoglycaemia, and regular monitoring. We complete the diabetes sections of the D4 form based on your current management.
I have sleep apnoea — what do I need?
The DVLA requires evidence of effective treatment (typically CPAP) and good compliance. Bring any clinic letters or compliance data. We can advise based on DVLA guidance on the day.
What if I have a heart condition?
Many heart conditions are compatible with a Group 2 licence once treated and stable. Bring any cardiology reports and a list of medications — Dr. Al-Hassan will assess based on DVLA standards.

Pricing

How much does a driver medical cost at SWDM?
HGV, LGV, PCV, and D4 medicals are £55. Taxi medicals are £95. Forklift and airside medicals are £70. Workplace drug tests are £20. Management referrals are £330. Full price list on our services page.
Do you charge if I fail the medical?
You still pay the examination fee regardless of outcome, because Dr. Al-Hassan's time and the DVLA form completion are the same whether you pass or need to re-examine after treatment.
